An opportunity has arisen for a Senior Practitioner to join the East Sussex Changing Futures Multi-Disciplinary Team in the Adult Social Care - Mental Health Service, the role is countywide with an office base at St. Mary’s House, Eastbourne. The post is being offered until March 2025 with the possibility of extension.
There are elements of office-based working, client visits and working from home. You will need to demonstrate you can meet the travel requirements of the role in your application.
Changing Futures Sussex was awarded a Changing Futures grant to improve the way local systems and services work for adults experiencing multiple disadvantages. The Programme aims to create an environment where individuals experiencing multiple disadvantages can receive flexible, trauma-informed, person-centred support when they need it, leading to increased periods of stability and more opportunities to make positive changes in their lives.
The Changing Futures Sussex Programme focuses on meeting the needs of the most vulnerable people who repeatedly fall through the net, working alongside people with lived experience of multiple disadvantages, to achieve positive changes in services to make them better connected and easier to access.
Multiple disadvantage, or multiple complex needs, is defined by the national Changing Futures Programme as people who experience three or more of the following: homelessness, current or historical offending, substance misuse, domestic abuse, and mental ill-health. The MDT includes a Practice Manager, Clinical Nurse, two Specialist Outreach Women’s Workers, Two Senior Substance Misuse Recovery Workers, Social Worker, Resource Officer, and Four Peer Navigators.
Experience we are looking for:
- Social work qualifications and registered with the professional regulatory body.
- Level 1 Practice Educator Professional Standards, equivalent or willingness to undertake.
- Supervision skills.
- Communicating with adults where there is complexity.
- Managing workload independently.
- Demonstrable effective judgement, identifying, assessing and managing risk.

Your starting salary will be pro rata if the above position is part-time or term-time only.
