East Sussex is a fantastic place to live and work, with its rolling hills and countryside, and green walks to small coastal towns. We are looking for an exceptional Resource Officer to come to work within our Adult Social Care Operational Teams based at Conquest Hospital, St Leonards-on-Sea.   

We have excellent relationships with local partners and hospital trusts. Together we work collaboratively to serve people to safely discharge from hospital back to their homes in the community. We promote the home first model and discharge to assess approach and believe with the appropriate safe level of care clients can where possible, maintain their independence and stay at home. We work closely with our reablement teams and our inpatient rehabilitation providers to maximise a client’s independence and build confidence and positive outcomes for the clients we work with.  

The resource officer post offers the opportunity to work in partnership with adults, their families, and carers, to enable them to identify the support and outcomes which they would like to achieve. Also, you will be supporting safeguarding duties to protect and mitigate risks to the clients we serve.

To carry out your role, you will need to be confident and competent or be willing to acquire the knowledge, in assessing risk, undertaking Care Act (2014) compliant assessments of an individual’s care and support needs, including mental capacity assessments and to have knowledge of Safeguarding Adults or be willing gain this knowledge to develop in these areas using a strength based, outcome focussed approach.

Your starting salary will be pro rata if the above position is part-time or term-time only.
