Are you looking for a job that is challenging and rewarding, that involves you working with different people every day and supports vulnerable adults with care needs to live independently in the community without needing to provide the hands-on care?

As an Extra-care coordinator you would work across the six Extra Care sites in East Sussex, working with various stakeholders to ensure the services are used effectively by:

  • Promoting the service
  • Supporting to identify potential appropriate referrals.
  • Co-ordinating new allocations and overseeing the waiting list
  • Ensuring voids are kept to a minimum
  • Monitoring and reviewing service provision
  • supporting future developments within the extra care and bedded care arena.

We are looking for candidates who:

  • are excellent communicators, skilled at building relationships and working in partnership with a range of stakeholders.
  • can develop, collate and analyse data
  • have experience of working in Adult Social Care and an awareness of the relevant legislation relating to housing and social care
  • are organised and self motivated

Additional Information

Your starting salary will be pro rata if the above position is part-time or term-time only.

Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
