NHS and Social Care – working together in East Sussex

Adult social care information leaflets and factsheets are designed to give you basic information about our core services. They have been approved by the Plain English Campaign for clarity.

We have just three main information leaflets (IL1 to IL3). These are also available in an 'easy read' format.

We also have 12 factsheets, FS1 to FS10 and FS13. (Please note, FS11 – Direct payments to suitable persons is currently in production.)

The factsheets give more detail, including the way we charge for services.

The titles and reference numbers below link to where you can download, read and print each leaflet or factsheet.

But if you prefer to be sent a printed version, please contact Social Care Direct (quoting the relevant reference number such as IL1, IL2 or FS1).

Who are they for?

Our leaflets are for anyone interested in the services we provide. You may be applying for services yourself, or arranging services for someone you care for.

If you work for an organisation, you may want to keep copies of these in your leaflet racks for your customers.

When are they updated?

To make sure that we keep you informed of our services, we aim to update these leaflets every year, usually around spring.

The leaflets and factsheets were last updated in May 2010.

Can I get them in other formats or languages?

We want our leaflets and factsheets to be accessible to everyone. You can get all of them in the following formats:

How can I get copies?

You can get printed copies of our leaflets and factsheets by contacting Social Care Direct.

Or, if you want more than one copy, please complete our information leaflet order form.

Make sure you let us know:

  • who you are and where you would like them delivered
  • how many copies you want and in what format
  • the title and reference number of the leaflets or factsheets you want.
