Closer to Home – joint planning for health and Adult Social Care
This information is aimed at the Primary Care Trusts (PCT's) we work with.
In autumn 2007, we held five Closer to Home events across the county, in partnership with:
- Hastings & Rother Primary Care Trust (PCT)
- East Sussex Downs & Weald PCT.
These events were the first of their kind in East Sussex – with the two PCTs and Adult Social Care coming together to talk about health and social care issues with people from the local areas.
